Notice of Consumer Access to Health Records and Complaints

YOUR RIGHTS REGARDING HEALTH RECORDS

In accordance with applicable federal and Texas law, patients have the right to request access to or copies of their health care records held by this office.

To request your health records, please:

1. Call our office or submit a written request to release your records.

2. Complete any required authorization forms.

3. Provide valid identification.

4. Specify the dates of the requested records and, if applicable, where they should be sent.

Requests for records may be submitted in person, by mail, fax, or email, as permitted by law.

Please contact the Curis Functional Health office where you were treated for assistance with record requests, applicable fees, processing times, or questions about your protected health information.

If you have concerns about your care, privacy rights, billing practices, or access to records, you may contact the appropriate licensing or disciplinary authority listed below.
